The priority deadline for admissions and financial aid applications has passed, but we will continue to welcome, accept, process and review applications to all grades (PK-12) for enrollment consideration. Blake's application for admission fee is $75.00 before December 1 and $100.00 after December 1. In some cases, application fee reductions are available; please call the Financial Aid Office at 952-988-3424 for more information.

Step 2: Print the form - please click on the printer icon at the top or select "File" and then "Print" on the menu bar at the top of your screen.

Step 3: Complete Admission forms.
